Een man raakt seksueel geobsedeerd door een DVD opname van de eerste film van de "Human Centipede" serie. Hij gebruikt schuurpapier om zichzelf te bevredigen tijdens het bekijken van de film. Hij besluit een eigen "Human Centipede" te maken van 12 slachtoffers en zijn eigen 'centipede' te gebruiken om zijn eigen seksuele genot te bevredigen.
